In some situations, I don't put a mesh. I weave it with the same suture material, a procedure called DARNING which is equally strong and effective.

We have to gently push the hernia and it's contents back into the abdomen
The hernia has gone back into the abdomen, it's now ready for the repair
The MESH is placed overlying and covering the entire repair to reinforce it
Once the MESH is fixed, the wound is closed in layers
Whatever difficulty we might have encountered, you are left with a clean wound at the end
